Dear Parent/Guardian,

Your child was recently given a form for you to fill out and return that asks
 for your child’s race and ethnicity. The collection of this information is mandated
by the federal government, which has revised the criteria of these data. A letter of
explanation was included with the form but more information can be found
on the PWCS Web site by clicking here.

Pease complete parts A and B of the form, sign and date it at the bottom, and
return it to your child’s teacher by Thursday, March 18.

As called for in the new federal requirements, if the forms are not completed by
parents/guardians, then a staff member will observe and select racial and ethnic
categories on the student’s behalf.
